Identifying Vulnerable Patients

“As Practice Manager for a busy local GP Surgery, I was able to utilise the 111 screen within the Secondary Care dashboard to identify frequent callers.  There was an individual in particular who upon investigation was found to be a young, new mother that was struggling emotionally and in need of support.  I worked alongside the patient’s health visitor, the nurses within my Practice and associated services to ensure that the patient was properly cared for by the surgery and the community.  Within a few weeks, the young mother was no longer calling 111 for help and has a stronger support network as a result.”
