Identifying High Cost Patients
A local practice identified, using RAIDR’s Secondary Care dashboard a significant frequent user of Accident & Emergency services. They located the patient’s records on their Practice’s clinical system for review and began the process of establishing a mutually agreed care plan to reduce the attendance rates and pressure on emergency services while improving the patient’s outcome. The Practice aims to reduce the attendances from 100 per annum to less than 25. RAIDR estimated the cost of that particular patient’s attendances at circa £85,000 per year at current levels therefore the Practice predicts implementing similar interventions could reduce costs by approximately £50,000 per year.
