Cultural change – supporting organisations to develop their infrastructure to enable population health approaches to be embedded to support decision making.
Community of practice – support to build a thriving and exciting community of practice locally, by:
- Establishing networks of stakeholders
- Hosting regular webinars
- Sharing videos / blogs
- Facilitating discussion both online and in person
- Celebrating successes
- Sharing lessons learned.

Our experience
A comprehensive PHM programme was delivered across Humber & North Yorkshire to address health inequalities and improve health outcomes for the population. As part of this programme:
- Training and education sessions were delivered to health and social care professionals and associated resources developed.
- 25 workshops were delivered to over 230 staff.
- 19 training sessions were delivered.
- A community of practice was established to share best practice.
- Four training videos were produced.
