Our offer

We can support you with building capability and confidence in your workforce to deliver population health with our training and coaching offer. We can:

  • Provide advice and support to Executive Boards to embed a population health approach in organisations across your system.
  • Develop and deliver training programme for health and social care staff and organisations to build their knowledge and confidence in this area.

We can provide training and education sessions and video resources, coaching, mentoring and development programmes to support embedding a population health approach to support:

Board level development, by providing advice and support to Board members to develop their leadership in population health management.

Build capability and capacity, within organisations / systems by the introduction of Population Health for Leaders by offering:

Expertise and solutions to organisations to build confidence and sustainability around population health delivery, including:   

  • Bespoke development programmes
  • Peer action learning sets
  • Specialist and expert coaching​.

Cultural change​supporting organisations to develop their infrastructure to enable population health approaches to be embedded to support decision making.

Community of practice –support to build a thriving and exciting community of practice locally, by:

    • Establishing networks of stakeholders
    • Hosting regular webinars
    • Sharing videos / blogs
    • Facilitating discussion both online and in person
    • Celebrating successes
    • Sharing lessons learned.

    Our experience

    A comprehensive PHM programme was delivered across Humber & North Yorkshire to address health inequalities and improve health outcomes for the population. As part of this programme:

     

    • Training and education sessions were delivered to health and social care professionals and associated resources developed.
    • 25 workshops were delivered to over 230 staff.
    • 19 training sessions were delivered.
    • A community of practice was established to share best practice.
    • Four training videos were produced.
