Our experience
A comprehensive PHM programme was delivered across Humber and North Yorkshire to address health inequalities and improve health outcomes for the population. As part of this programme:
- Training and education sessions were delivered to health and social care professionals and associated resources developed.
- 25 workshops were delivered to over 230 staff.
- 19 training sessions were delivered.
- A community of practice was established to share best practice.
- Four training videos were produced.
