Before Lord Darzi’s report on the state of the National Health Service in England, the government set out three strategic shifts for the NHS: moving care from hospitals to the community, embracing digital transformation and shifting from treatment to prevention.

Integrative Role of PHM

PHM acts as a bridge across these three shifts by leveraging data-driven insights, multi-disciplinary collaboration and technology to:

  • Address population needs holistically.
  • Prioritise equity by focusing on vulnerable groups.
  • Deliver tailored interventions that improve individual and community health outcomes.

Through this lens, PHM is not just a management tool but a transformative approach that aligns seamlessly with the visionary goals of the Darzi framework.

 

Our offer:

Our comprehensive offer will support you to develop capability to embed Population Health Management.

We will train staff to understand population health management as well as providing facilitated support working with systems and communities, to guide people through the whole cycle or part of the cycle.

We can also provide support with:

  • Developing strategic documents and health needs assessments making use of PHM techniques and action plans (such as transformation plans and health inequalities action plans).
  • PHM approaches being adopted as part of national planning asks, such as ICC development and INT development.

We do this by using:

 

  • Data visualisation using for example our community of 1000 report
  • Priortisation matrix to decide which areas to focus on first
  • Process mapping and theographs to understand patient journeys
  • Stakeholder mapping to ensure all partners are involved in the PHM journey
  • Gathering the patient voice for example via questionnaires
  • Coaching and mentoring
  • Facilitated workshops and discussions for genuine collaboration and better solutions designed together
  • Facilitation and support to develop action plans
  • Ideas generation and review of evidence – consideration of how interventions can be tailored and targeted to meet needs of particular groups.

Our experience

Below are examples of where we have successfully used these approaches:

 

  • NENC ICB Clinical Strategy – PHM approaches helped in developing priorities and considering HI
  • Medway PNA – PHM approaches were utilised in understanding pharmaceutical needs and making recommendations for the future.
  • Humber and North Yorkshire – PCNs and places received facilitated support to help understand their population’s health needs using linked datasets, intervention design and action plan development – in some areas including INT development.
  • Coventry Peri-natal Mental Health Needs Assessment.
  • North Tees & Hartlepool NHS FT DNA social prescribing project.
