Our experience
Below are examples of where we have successfully used these approaches:
- NENC ICB Clinical Strategy – Population Health Management approaches helped in developing priorities and considering HI.
- Medway PNA – PHM approaches were utilised in understanding pharmaceutical needs and making recommendations for the future.
- Humber and North Yorkshire – PCNs and places received facilitated support to help understand their population’s health needs using linked datasets, intervention design and action plan development – in some areas including INT development.
- Coventry Peri-natal Mental Health Needs Assessment.
- North Tees & Hartlepool NHS FT DNA social prescribing project.
