The challenge
NHS Derby & Derbyshire ICB has monthly Community Disciplinary meetings with GP Practices. It was agreed that they would look to identify frequent A&E attenders from the surgeries covered, to support anyone who was making unnecessary visits to the service.
Our response:
Utilising our RAIDR Patient Activity dashboard, NHS Derby & Derbyshire ICB identified any patients that had attended A&E five times or more, using the A&E Frequent Users screen. This enabled the team to identify a cohort of patients, and practice staff were able to dynamically look at the reasons why they were attending – providing valuable insight.
Outcomes
This is an ongoing piece of work that NECS is linked in and collaborating with. Outcomes are being reviewed on a regular basis.
